TOOTH FAIRY INC PROGRAM GUIDELINES & INSTRUCTIONS
Tooth Fairy Inc is a non-profit corporation dedicated to serving both foster and economically disadvantaged children in Southern Idaho. The Tooth Fairy Program was established to facilitate orthodontic care for foster children and other children referred through partner non-profit organizations. In order to be considered for funding, the following must be completed:
- Request for funds application completed in full (missing statements will be cause for rejection)
- An itemized quote from the orthodontist
- A written statement from the case worker (if applicable) confirming the child’s need for orthodontic care
- Photo of smile, frontal and side view (a cropped photo of the child’s smile only is acceptable)
- Letter from school dministrator/counselor/teacher/pastor indicating his/her observations of the child being introverted or severely lacking in confidence or that child is a target of bullying as a result of his/her dental issues.
- Recipients of funds will be required to submit a biannual report from the orthodontist confirming that appointments have been kept and that child is practicing good oral hygiene. A form will be sent out once award has been made.
Once all materials have been received, please expect 2 -3 weeks for a decision to be made. Tooth Fairy Inc may award funds for partial or full payment of dental care depending on funds available. All payments will be made directly to the provider.
